Birth and Death Certificates

BIRTH CERTIFICATE CLIENTS

• Births are on file at the health department from 1940 to present (A birth certificate needed for someone who was born before 1940 must be obtained from the Division of Vital Records, Baltimore, Maryland. The fee is $12.00 when ordering from Baltimore )
• The Health Department charges a fee of $20.00 a record (Sorry, we cannot accept out of area checks)
• Must have acceptable Photo Identification. Examples include:

  Department of Motor Vehicles photo identification cards

  Driver's license

  United States Passports

  NO SIDE VIEW IDs ACCEPTED

  ALL IDs must have an issue and expiration date-NO EXCEPTIONS.
  If your ID does not include a full middle name, you will be required to show a second form of identification. We reserve the right to refuse any ID that we deem questionable. Must be listed on birth record to obtain copy (father, mother or individual).
  All legal documents must be sent to Baltimore:
  

  • Court orders
  • Power of attorney
  • Notarized letters

    WE CANNOT ACCEPT ID'S THAT ARE EXPIRED, CRACKED, CHIPPED, BROKEN, FRAYED OR FADED.

If you need copies of records, please call (301) 759-5002 or come to the Allegany County Health Department, 12501 Willowbrook Road, SE, Cumberland, Maryland. We are open Monday through Friday from 8 a.m. to 5 p.m. Birth and death certificate applications are accepted from 8:00am until 3:30pm.

PLEASE NOTE: If you are planning to travel outside the United States, you must obtain a certified copy (with raised seal) of your Maryland birth certificate. You may not substitute a photocopy of same, nor a birth registration notice, Statement of Age card, nor a hospital-generated birth record.
