Birth and Death Certificates

  • Birth certificates are available at the Allegany County Health Department for births from 1940 through present, for a fee of $20. (Cash and local checks accepted.)
  • Birth certificates for years prior to 1940 must be requested from Baltimore, for a fee of $12 by mail.
  • Birth certificates may only be obtained by those persons who are named on the certificate; i.e. mother, father, individual.
  • All birth certificate requests involving court orders, powers of attorney, or notarized letters must be through the Baltimore office.
  • Proper photo identification is required --
  • Driver's License,
    United States Passport,
    Dept. of Motor Vehicles photo ID cards

    • No side view IDs are accepted.

    • IDs must include an issue date and an expiration date.

    • If the ID does not include a full middle name, an additional form of identification will be required; i.e. Social Security card.
    • IDs THAT ARE EXPIRED, CRACKED, CHIPPED, BROKEN, FRAYED, FADED, OR PEELING WILL NOT BE ACCEPTED.
    • We reserve the right to refuse any ID that WE deem questionable.

IMPORTANT: If you are planning to travel outside the United States, you must obtain a certified copy (with a raised seal) of your Maryland birth certificate. The Allegany County Health Department issues certified copies. You may not substitude a photocopy of same, a Birth Registration Notice, a Statement of Age card, or a hospital-generated birth record. Death Certificates

  • Death certificates are issued locally for 30 days following a death, for a fee of $20. (Cash or local checks accepted.)
  • After 30 days, the death certificate must be requested from the Baltimore office of Vital Records.
  • Death certificates are only released to a surviving relative or legal representative.
  • The deceased's name, date of death, and place of death are required.
