Evaluation and
OVERVIEW OF PROGRAM: Adult Evaluation and Review Services (AERS) initially was known as Geriatric Evaluation and Review Services when the program originated in 1974. In Allegany County AERS also serves as the lead agency for the Senior Care Program and the Family Caregiver Support Program. The AERS programs collaborate with various community and state agencies including Western Maryland Health System, Allegany County Department of Social Services, Allegany County Housing Authority, Human Resource Development Commission, Area Agency on Aging, area nursing homes and adult day care facilities. SERVICES:
The AERS, Senior Care, and Family Caregiver Support Programs conduct
in-home and on-site visits to evaluate individuals and refer them
for services they may be eligible to receive. ELIGIBILITY:
AERS is able to evaluate clients age 21 and older who have a disability
or a need for the services AERS provides. HOW TO OBTAIN SERVICES: Referrals are taken from other agencies, the client, family, or any concerned individuals. Usually an appointment is made by phone and a nurse or social worker will conduct an in-home or on-site visit to assess the client's needs. CONTACT:
